Group Adult Foster Care Program (G.A.F.C.)
The purpose of the Group Adult Foster Care Program (G.A.F.C.) is to provide personal care services to eligible MassHealth members who reside in the comfort of their homes. The program is designed to assist elderly and disabled individuals who need daily assistance with personal care.
WHO IS ELIGIBLE
» Be income eligible for Mass-Health Standard.
» Live in an affordable housing development.
» Need daily assistance with personal care such as bathing, dressing, grooming.
» Be approved by a physician for G.A.F.C. services.
» Be approved by Mass-Health for G.A.F.C.
» Have access to a working phone.
WHAT SERVICES CAN I GET AS A MEMBER?
- Assist with daily personal care such as bathing/shower, dressing, nail care, foot care, hair care, shampoo, skin care, oral care and more.
- Assist with Elimination.
- Assist with Ambulation.
- Assist with Meal Preparation.
- Assist with Grocery Shopping.
- Assist with Light Housekeeping.
- Assistance with Medication Reminder.
Home Health Aide Services
Provides personal care and assistance with activities of daily living such as: bathing, dressing, care for hair and teeth, assisting with exercises, ambulation, transfers, and repositioning. Other duties may include changing bed linens, light cleaning, and meal preparation during visits.
Homemaker Services
Our Homemaker Services includes housekeeping, laundry, meal planning and preparation, and routine house-hold activities by a trained homemaker, only when the caregiver and client are physically unable to provide the service.
Companion Care Services
» Assistance with phone calls and correspondence
» Assistance with maintaining medication schedules
» Accompany clients to medical appointments, shopping or recreation
Live-in Companion Care Services
Our Live-in Companions offer the highest level of personal care and assistance on an uninterrupted basis. Our kind and caring staff take personal pride and find true joy in helping our elderly clients. This service combines the Companion / Homemaker and Personal Care Assistant duties.
Personal Care Assistant Services (P.C.As)
Our PCAs assist patients/clients with Feeding, Grooming, Dressing, Bathing, Incontinence, Mobility, Medication reminders and Cognitive impairment.
Hospice Caregiver Services
We bring compassion and sensitivity to those suffering from a terminal illness. Many of our caregivers have in-depth experience with hospice care. They possess a very special ability to attend to your loved one’s needs during their final days.
Respite Care
We offer a unique program called Short-Stay Respite. Your loved one may want to take advantage of this option for several reasons:
